3. Proof of Immunization
In accordance with University System of Georgia Board of Regents
policy, all entering University of Georgia students born after 1957
are required to submit to the University Health Center a Mandatory
Certificate of Immunization form prior to registration for classes.
This form certifies immunity to mumps, measles, and rubella (MMR)
and to varicella (chicken pox). It also certifies that the student
has a current tetanus shot.
You should complete the Mandatory Certificate of Immunization form
and submit it to the University Health Center well before the start
of the semester in which you plan to enroll at UGA. The form should
be mailed to this address:

The form may also be faxed to the Patient Registration and Health
Information Office at 706-542-4959.
Please be aware that you will not be permitted to register for classes
without having certified your immunization. The University Health
Center on the main UGA campus in Athens provides the vaccination;
County Health Departments also provide the vaccination. Contact
your local County Health Department for MMR vaccination information.
